Defect: ON CERTAIN FRAME AND FRAMELESS DUMP TRAILERS, THE 8-INCH FIVE STAGE TELESCOPIC CYLINDER COMMONLY REFERRED TO AS A "SUPER DUMP", WITH USE THE PACKING NUT ON THE FOURTH STAGE RETAINING THE PLUNGER CAN LOOSEN, BACKING OFF THE FOURTH STAGE. THIS CONDITION, IF NOT REMEDIED, CAN CONTINUE UNTIL THE PACKING NUT BACKS COMPLETELY OFF THE STAGE. IF THIS OCCURS, THERE IS NOTHING TO RETAIN THE FIFTH STAGE ( (excerpt)
Consequence: WHEN THE CYLINDER IS EXTENDED TO ITS MAXIMUM STROKE, THE FIFTH STAGE CAN SEPARATE FROM THE CYLINDER ASSEMBLY CAUSING LOSS OF CONTROL OVER THE TRAILER, ALLOWING THE TRAILER TO FREE FALL, POSSIBLY CAUSING SERIOUS INJURY OR DEATH. (excerpt)
Remedy: PARKER HANNIFIN WILL PROVIDE A FIELD MODIFICATION KIT #375 1805 206 TO SECURE THE PACKING NUT IN POSITION FREE OF CHARGE. OWNERS WHO DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT PARKER HANNIFIN AT 1-888-231-7461 OR MAC TRAILER AT 330-823-9190. (excerpt)

Notes on this record
Complaints from the API and bulk file are merged and deduplicated by ODI number; the API alone misses records.
System and subsystem counts are numbers of distinct complaints, recalls or communications that name them. One record can name several systems, so shares across systems can add to more than 100 percent; a single share never exceeds 100 percent.
Recalls are counted once per campaign, investigations once per action and manufacturer communications once per NHTSA id, even when NHTSA lists several model names for the same record.
A sales denominator is a calendar-year proxy, not the population of vehicles still in use. Rates are copied from verified pipeline output.
