Defect: ON CERTAIN TRANSIT AND SCHOOL BUSES, A BRASS FITTING CONNECTS THE DOUBLE CHECK VALVE TO THE BRAKE RELAY VALVE. WITH CERTAIN INPUTS TO THE VEHICLE, THE WEIGHT OF THE CHECK VALVE ON THE FITTING CAN CAUSE THE ASSEMBLY TO RESONATE. THE SUBJECT VIBRATION CAN CAUSE EXCESSIVE STRAIN LEVELS IN THE FITTING, WHICH COULD EVENTUALLY FATIGUE AND FAIL. (excerpt)
Consequence: IF THE FITTING BREAKS, THE REAR SERVICE BRAKES WILL CEASE TO OPERATE WITHOUT WARNING, RESULTING IN AN EXTENDED STOPPING DISTANCE, INCREASING THE RISK OF A CRASH. (excerpt)
Remedy: DEALERS WILL INSTALL A KIT TO REMOUNT THE DOUBLE CHECK VALVE WITH A FLEXIBLE HOSE ON THE AMJORITY OF VEHICLES. ON A SMALL NUMBER OF UNITS WITH CLEARANCE ISSUES OR TRACTION CONTROL, A KIT WITH A NEW RELAY VALVE THAT HAS A BUILT-IN DOUBLE CHECK VALVE WILL BE INSTALLED. OWNER NOTIFICATION BEGAN AUGUST 30, 2002. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE (excerpt)
Defect: VEHICLE DESCRIPTION: SCHOOL AND TRANSIT BUSES EQUIPPED WITH 39-INCH FLIP SEATS. THE FLIP SEATS COULD BIND IN THE OCCUPIED POSITION AND NOT RETURN TO THE STORED POSITION WHEN NOT OCCUPIED. THE CUSHION FRAME SUPPORT BAR SLIDES OFF THE SEAT SUPPORT PAD, WHICH COULD RETAIN THE SEAT IN THE OCCUPIED POSITION.
Consequence: THE SEAT COULD BLOCK THE AISLE TO THE EMERGENCY EXIT DURING AN EMERGENCY, WHICH MAY RESULT IN INJURY OR DEATH.
Remedy: OWNERS WILL BE PROVIDED WITH INSTRUCTIONS AND PARTS TO REPAIR THE SEATS FREE OF CHARGE. OWNER NOTIFICATION BEGAN OCTOBER 23, 2001. OWNERS WHO DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT AMTRAN AT 800-843-5615.
Do not drive: No. Park outside: No.

Notes on this record
Complaints from the API and bulk file are merged and deduplicated by ODI number; the API alone misses records.
System and subsystem counts are numbers of distinct complaints, recalls or communications that name them. One record can name several systems, so shares across systems can add to more than 100 percent; a single share never exceeds 100 percent.
Recalls are counted once per campaign, investigations once per action and manufacturer communications once per NHTSA id, even when NHTSA lists several model names for the same record.
A sales denominator is a calendar-year proxy, not the population of vehicles still in use. Rates are copied from verified pipeline output.
